Overview

The LPN position will coordinate the care for all patients assigned to a designated team.  He/she will review information related to a patient’s present illness, diagnostic reports, recent inpatient, outpatient, and/or ancillary services received in a setting other than the clinic.  This person will utilize EHR Population Health Management tool (currently Care Manager) to identify care gaps that providers will be prompted to address.  This person will be responsible for leading morning and mid-day pre-clinic huddles to prepare all providers and clinical support staff for needs/challenges that are anticipated.  This person will be responsible for scheduling procedures that clinic providers perform at off-site clinical locations, to include colonoscopies, endoscopies, and stress tests. Maintain good working relationships with patients, physicians and employees.  This person will be required to fill in for absent nurses and/or perform assigned administrative or other patient care duties, and will be required to float between both clinics (Springdale and Fayetteville).  This position will require rotation through the night clinic.

Responsibilities

Key Responsibilities (list primary functions of the position with the percent of time generally allocated to each; percent of time must equal 100%)

 

70% CARE COORDINATION

  • Works in collaboration with multi-disciplinary team to provide support to medical providers and clinic staff.
  • Initiates and maintains communication between providers, support staff, and outside resources.
  • Serves as a resource to clinical staff and providers, as needed.
  • Performs pre-visit planning for all clinic visits to occur within 72 hours.
  • Leads morning and midday huddles to review patient needs and preventive care services/care gaps identified in pre-visit planning, to include diabetic foot exams, Medicare Wellness Visits, immunizations, labs, etc.
  • Provides documents needed in the office visit (PMP’s, depression screening, immunization records, hospital records, etc.
  • Works with local hospitals to identify clinic patients who had an emergency room or inpatient visit and reviews discharge documents.
  • Processes Medicaid ED and Urgent Care Clinic referrals when appropriate.
  • Contacts patient within 72 hours of hospital discharge (1 week for ED visits) to evaluate status, review and reconcile discharge medication list with patient, and confirm or schedule follow-up appointment within 7 – 14 days post discharge depending on risk stratification.
  • Coordinates effective Transitions of Care, to include: transitions between primary care, specialists, and facilities.
  • Covers for other coordinators when necessary.

 25% PATIENT CARE

  • Assists with the triage of urgent matters that present via phone or walk-in patients and when immediate assistance is required.
  • Checks and responds daily to messages received via the patient portal.
  • Checks and completes daily to electronic refill requests (eScribe) per standing orders.
  • Schedules Nexplanon insertions and manages inventory.
  • Schedules procedures that clinic providers perform at off-site clinical locations, to include colonoscopies, endoscopies, and stress tests.
  • Covers for floor nurse and or/ Nurse Manager during vacations, illness, or staff shortage.

 5% MISCELLANEOUS DUTIES

  • Attends staff meetings, trainings, and in-services, as assigned
  • Participates in Quality Improvement activities.
  • Promotes positive customer (both internal and external), provider, and co-worker satisfaction and teamwork atmosphere.
  • Other duties, as assigned.

Qualifications

Required: Graduate of an accredited Nursing Program is required. Valid LPN License, minimum of 0-6 months of LPN experience; excellent communication skills, strong work ethics, basic computer skills, and accountability.

 

Preferred: 2 years or more nursing experience with current BLS certification.
