  • Overview

    The RN Case Manager will apply expert knowledge, skills and experience to provide case management through critical thinking. Works closely with the appointment center team, all OB clinics, and referring physicians and health departments around the State.

     

    Responsibilities

    Applies expert knowledge, skills and experience to care for patients with complex diagnosis. Assist Director in assuring optimal operational activity through review of Telemedicine patient records, providing assistance for appointment center staff questions and referral placement. Promotes positive interaction and facilitates communication between the ANGELS Program, CDH, OB GYN department and referring entities. Promotes positive interactions to improve patient outcomes at all levels of the organization. Ensure the betterment of the ANGELS programs telemedicine clinical delivery/appointment scheduling/and outreach.

     

    Qualifications

    Required: Graduate of an accredited Nursing Program with Associates Degree, Nursing Diploma, Bachelors in Nursing, or Master's in Nursing.  Valid RN License,  must have a minimum of 3 years of RN Experience in Women's Health.  Excellent communication skills, basic computer skills, strong work ethics and accountability.

     

    Preferred: 4 years or more nursing experience in Women's Health with current BLS.
