The Licensed Practical Nurse (LPN), is a specifically trained and licensed member of the Walker Family Clinic team. The LPN supports the on-going delivery of care, as identified by the multi-disciplinary scope of practice. The LPN works with the individual patient, their families, and providers of care to implement individual health plans, under the direction of the RN.
Schedule: Full-Time, 8:00am-4:30pm
The LPN will provide patients support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state and will work with the individual patient, their families, and providers of care to develop and implement individual health plans, and provide support to the patient in self-management. Coordinate care and/or follow-up for patients who receive care in outpatient facilities. Coordination of prescription refill requests and patient education coordination in collaboration with RN’s and Physician staff. The LPN will develop a specialty in patient care related to the clinics populations and acts as a resource. The LPN will work with the providers and clinical staff to implement innovative approaches and set priorities to meet patient/family needs. Required: Graduate of an accredited Nursing Program is required. Valid LPN License, minimum of 1 year of LPN experience; excellent communication skills, strong work ethics and accountability, basic computer skills.
Preferred: 2 years or more nursing experience with current BLS certification.
