The Licensed Practical Nurse (LPN) is a specifically trained member of the Family Medical Center team. The LPN supports the on-going delivery of care, as identified by the multi-disciplinary scope of practice. The LPN works under the direct supervision of the Registered Nurse. The LPN works with the individual patient, their families, and providers of care to implement individual health plans.
The LPN will provide patients support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state and will work with the individual patient, their families, and providers of care to develop and implement individual health plans, and provide support to the patient in self-management. Coordinate care and/or follow-up for patients who receive care in outpatient facilities. Coordination of prescription refill requests and patient education coordination in collaboration with physician staff. The LPN will develop a specialty in patient care related to the clinics populations and acts as a resource. The LPN will work with the providers and clinical staff to implement innovative approaches and set priorities to meet patient/family needs. Commitment to patient outcomes results in high levels of performance despite workload changes.

Required: Graduate of an accredited Nursing Program is required. Valid LPN License, will consider New Graduates with BLS Certification; excellent communication skills, strong work ethics and accountability, basic computer skills.
Preferred: 1 year or more nursing experience with current BLS certification.
