  • Overview

    Schedule: Part-Time, Days. 

     

    The Registered Nurse (RN) will promote wellness; improve outcomes, and self-care within a defined population of patients in the Medical Onoclogy Clinic.  The RN will provide patients who have chronic conditions the support, education, and assistance in the prevention and/or maintenance of their disease and/or health and wellness state.  Will work with the individual patient, their families, and providers of care to develop and implement individual health plans and provide support to the patient in self-management.  Coordinate care and/or follow-up for patients who receive care in outpatient facilities.  Coordination of prescription refill requests and patient education coordination in collaboration with physician staff.  Commitment to patient outcomes results in high levels of performance despite workload changes.

Coordinate care and/or follow-up for patients who receive care in outpatient clinics. Coordination of prescription refill requests and patient education coordination in collaboration with physician staff. The RN will develop a specialty in patient care related to the clinic's populations and acts as a resource. The RN will work with the providers and clinical staff to implement innovative approaches and set priorities to meet patient/family needs. Commitment to patient outcomes results in high levels of performance despite workload change.

    Qualifications

    Required: Graduate of an accredited Nursing Program with Associates Degree, Bachelors in Nursing, Master's in Nursing, or Nursing Diploma is required. Valid RN License, minimum of 1 year of registered nursing experience.  Excellent communication skills, strong work ethics and accountability, basic computer skills.

     

    Preferred: 2 years or more nursing experience with current BLS.
